Most of the people supported by care workers in this region live with at least one chronic condition: diabetes, heart disease, respiratory illness, arthritis. This nationally recognised skill set trains support workers to help clients manage those conditions themselves: understanding the condition, supporting the care plan, and knowing when something needs escalating.
Self-management support is a distinct skill. Done well, it keeps people healthier at home for longer, which is the whole point of community care.
What's involved
Training and assessment against the skill set's units, centred on addressing the needs of people with chronic disease within a person-centred support framework. On successful completion you receive a Statement of Attainment for the units achieved, with credit available toward relevant community services qualifications.

Support workers in aged care, disability and community services; teams in home care organisations where chronic condition support is daily work. Ask us about group delivery for your workplace.
Fees and funding
Eligible Queenslanders can access this skill set subsidised under the Career Boost program at the co-contribution fee shown above. Skill sets are exempt from Career Boost's prior-qualification rule, and each person can access up to two subsidised skill sets. Check your eligibility. If you're building toward a qualification, this skill set credits into the CHC33021 Certificate III in Individual Support.
